The basic difference is that in fragmentation, an organism breaks into pieces and each piece develops into new individual. In regeneration if organisms breaks into pieces, each piece may or may not develop into new individual.

Fragmentation is limited to invertebrate forms while regeneration is present in both vertebrates and invertebrates.

Fragmentation is a method of asexual reproduction used by the invertebrates (sponges,starfish etc.) Regeneration can be used as a reproduction method (e.g. Star fish) or it can be used to regenerate lost body parts (e.g. Lizards).

Regeneration is a modified form of fragmentation.
