The transition state of E2 consists of an H atom and a leaving group from the substrate aligned in the plane. The removal of proton and leaving group are in anti-periplanar. Therefore, in E2 elimination occurs in anti-periplanar geometry.

The given fischer projection should be converted to Sawhorse projection to analyse the anti-periplanar geometry.

The sawhorse projection does not have Br and H in anti-periplanar geometry so the molecule undergoes C-C bond rotation to make Br and H in anti-periplanar.

In this rotation of the molecule, both CH3 molecules comes in syn position. Thus, the elimination of Br and H gives cis-alkene product.
