The grandfather of someone will be the father of his father. So in the question when Q is a doctor and father of T and U is a contractor and grandfather of R.

If we are asked about the sister of T it is obvious that T being the grandchild of U, T is also a sibling to R as U is the grandfather of R.

So option A is the correct answer.
