Refraction takes place when light rays enter from one medium to another. Here we have two mediums, one is air and the other is glass. At first the light ray enters from air into the glass and refraction takes place. Again when the light ray enters air from the glass, refraction takes place. So in totality, the phenomena of refraction is observed twice.
