The correct option is B a point charge
Potential at a distance r from a point charge q is

V=kqr

For the equipotential surface V should be constant throughout the surface and for this given case, it is satisfied for spherical surface where r= constant at surface of sphere.


The equation of equipotential surface x2+y2+z2=4 represents a sphere with r=2. So, it will only be possible when source is point charge.
