09:00 am in the 24-hour clock = 09:00 hrs
Span between his first dose and third dose = 2 × 5 hours = 10 hours

10 hours after the first dose in 24-hour format = (09+10):00 hrs = 19:00 hrs
19:00 hours in 12-hour clock = (19-12):00 pm = 07:00 pm
