A test tube baby program aims to help infertile couples to fulfil their dreams of having a baby through a set of medical procedures. For it to be carried out successfully, a number of steps have to be followed.
The various steps in order of occurrence are ovarian stimulation→ gamete retrieval→ fertilisation→ embryo transfer.
Ovarian stimulation can also be referred to as superovulation. In this step, the ovary is given hormonal treatment to produce several mature ova instead of just one. LH surge induces the release of oocyte from the mature follicle.
In gamete retrieval step, the sperm and oocyte are extracted from the couple or donors. Once the hormonal treatment has brought about the production of several mature oocytes, the physician extracts the oocyte through a non- invasive procedure called transvaginal oocyte retrieval (TVOR). A thin needle, guided by sonogram, is inserted through the vaginal wall enters the ovaries to extract several mature oocytes. The collection of sperms is done in the form of semen from the male partner either by ejaculation in a collection condom or by surgical methods if there is an obstruction which prevents the normal ejaculation of the sperms through the urethral tract.
Then the gametes are made to fuse or fertilise. Fertilisation can either be done naturally or the sperm can directly be injected into the cytoplasm of the oocyte artificially.
After fertilisation, the embryo is implanted in the female genital tract for further development.
