The meaning of 'If they did the consequence would be terrible' is that if Sergius or Major Petkoff would have known that Catherine and Raina had saved a man from the Serbian army they would not only be in trouble for doing such an act, but also the consequences of it shall be terrible for them throughout their life as it shattered the romanticised notions as prevalent during that point of time.
