Osmotherapeutic agents are hypertonic solutions of low molecular weight that increase serum osmolarity, thus creating an osmotic gradient between blood and brain tissue. An intact blood–brain barrier is essential for this osmotic gradient. Migration of osmotic substances through the damaged blood–brain barrier can reverse the osmotic gradient and aggravate brain edema (rebound effect).
