In tubular secretion, are substances entering or leaving the bloodstream?

Tubules are the site of both secretion ion and reabsorption of substances in the nephrons. Secretion usually removes substances that are too large to be filtered (ex: antibiotics, toxins) or those that are in excess in the blood (ex: H+, K+) which no doubt leaves the bloodstream and goes to pct.
