A fuse wire may possibly melt when:

(i) the earth wire is broken:
the excess current that was supposed to be grounded through earth wire won't get a path to the earth. So this excess current will melt the fuse wire instead.

(ii) the fuse is fixed along the neutral wire instead of the live wire.
The fuse will melt anyway even if it is fixed in the neutral wire. However, it won't serve its purpose with safety in that case.

(iii) there is a short circuit in the electrical circuit:
Short circuit leads to a high current which melts the fuse.
