The transport of food, mineral ions and water over long distances via vascular tissues (xylem & phloem) is termed as translocation.
Loss of water by plants is termed as transpiration. It results in pulling of water along with mineral ions into the different plant parts such as leaves. This is called transpiration stream. So mineral ions & water are translocated via xylem, which is pulled by transpiration.
When ions are actively transported to the roots, it pushes water to move along the concentration gradient. This increases pressure inside the root i.e in the xylem present in the root called root pressure. However, root pressure is not enough to push water or minerals to all the way up to the leaves.
