Does the apparent position of the object just seem to fluctuate or does it actually fluctuate due to change in refractive index of hot air with respect to cold air?

Apparent means that it appears to. So, apparent fluctuation means that it appears to fluctuate. The position of the star is same in the sky, it appears to fluctuate to our eye because of the atmospheric refraction.
