Distance formula $=\sqrt{{(x-x_1)}^2+{(y-y_1)}^2+{(z-z_1)}^2}$ $$\begin{array}{cc}pQ& =\sqrt{(3-0{)}^2+(4-4{)}^2+(5-0{)}^2}\\ pQ& =\sqrt{9+0+25}\\ PQ& =\sqrt{34}\end{array}$$ Hence, (A) is the correct option.
