According to the tangent – secant theorem The product of secant segment and its external segment is equal to the square od the tangent segment drawn from the external point

$⟹T{R}^2=PR\times QR$

$=(PR+QR)\times QR$

$(5+4)\times 4=36$

$TR=6cm$
