Option B: The error lies in part (b): 'that had hurt herself'. The word 'that' is the error.
'That' as a relative pronoun is used in defining clauses, to describe a noun by giving its identity. For e.g. 'my sister that is sick is only 9 years old.'
When the clause is separated by commas ',' the clause is non-defining, i.e. it only adds extra information, without affecting the meaning and structure of the main sentence.
In non-defining clauses, such as this one, 'that' cannot be used to refer to persons or things.
Hence, 'that' is erroneous here.
The correction is 'who had hurt herself'.
Hence option B is the correct option with the error.
The other parts (a) and (c) do not have any error in them. They are grammatically correct and are complete in meaning.
