The verb 'discuss' itself means 'to talk about something' 'About' is already implied.. Hence the preposition 'about' when used again causes repetition: 'We talked about about the problem". Hence A is the correct option as it has the error.
Options B and C are grammatically correct and have no error in them. Hence we choose neither B nor C.
Option D is incorrect because the error lies in A.
