Given that

Perimeter $P=4x+20$

If square has a side $a$, we know that Perimeter $P=4a$

$\Rightarrow 4a=4x+20$

$\Rightarrow a=\frac{4x+20}{4}$

$\Rightarrow a=\frac{4(x+5)}{4}$

$\Rightarrow a=x+5$

We know that if $a$ is the side of square, then length od diagonal $D=\sqrt{2}a$

$D=\sqrt{2}a$

Substituting the value of $a$ in this above eqution we get,

$D=\sqrt{2}(x+5)$
