The correct option is C Medicine should be taken in time.

An action in a sentence can be represented in two ways, namely active and passive voice.

The passive voice always uses the past participle form of the main verb irrespective of any tense. Only the auxiliary verbs depend upon the sentence given in the active voice.

The correct answer is B)Medicine should be taken in time.

Option B is correct as the given sentence is an imperative statement with the implicit subject of 'you' (it is understood that the speaker is speaking to someone in front of him) , the transformed sentence also uses the simple present tense using the appropriate helping verb (should- because it is advisable and gives an indication of compulsion) with 'be' and the participle of the main verb (take).

Options A, C, and D are wrong as the transformed sentence cannot start with 'in' , the sentence structure is wrong, and the given sentence is not in the present perfect tense respectively.
