Thrombocytes, also called as platelets, release the chemical called thromboplastin at the site of injury. The thromboplastin further converts inactive prothrombin into Thrombin. Thrombin, in turn, converts inactive fibrinogen into fibrin. Fibrin forms the meshwork in which cells get trapped and now the big clump formed in called a clot.
