Here, if we number the compound clockwise, we get substituents at 2 and 3,3. If we number it anti-clockwise, we get them at 2 and 2. This preceeds the clockwise way of numbering and hence the name would be, 6-bromo-6-ethoxy-7-hydroxy-2-iodo-2,5-dimethylcycloheptanecarbaldehyde.
