At the site of injury, the ruptured tissue cells release thromboplastin/thrombokinase/Factor X which acts as an enzyme with Ca+ which converts prothrombin (inactive) into thrombin (active). Thrombin converts soluble fibrinogen into insoluble fibrin which forms a mesh-like structure to trap the blood cells forming a clot.
