a. 5x+6=0 can be written as 5x+0y+6=0
b. 2x+5y=0 can be writen as 2x+5y+0=0
c. 5x+y+xy=0 can be written as 5xy+y2x=0

This is not in general form as ax+by+c=0
d. y=6 can be written as 0x+1y6=0
So only option (c) is not in general form.
