The correct option is C $2 x + y + \frac{x}{y} = 0$
a. $5 x + 6 = 0$ can be written as $5 x + 0 y + 6 = 0$
b. $2 x + 5 y = 0$ can be writen as $2 x + 5 y + 0 = 0$
c. $5 x + y + \frac{x}{y} = 0$ can be written as $5 x y + y^{2} x = 0$

This is not in general form as $a x + b y + c = 0$
d. $y = 6$ can be written as $0 x + 1 y - 6 = 0$
So only option (c) is not in general form.
