how is the rate of heart beat determined from the ECG ?

Dear student,
In ECG we can see PQRST complex in which QRS complex is the tallest peak which is repeated at same rate. One QRS complex represents that one heart beat has taken place. In order to calculate the heart rate the space between the QRS complex is counted. One big square equals equals 1 unit and small square equals 0.2 unit. Therefore by counting the number of squares between the two QRS peaks the heart rate can be calculated.
